TERMS AND CONDITIONS
Once you have completed the information on this online form and submit it, you will be contacted by a representative within 24 hours to discuss your reservations. You will then receive a confirmation letter via email. Until you are sure of your plans, do not sign off with approval. Once you sign off with your approval, your credit card will be charged and penalties will apply .Please plan your trip carefully, because there will be a $25 Revision Fee per change from your original booking. No changes will be allowed within 7 days of departure. You will receive your vouchers and itinerary in the mail.

All refunds are subject to a $50.00 + 4.712% Hawaii General Excise Tax cancellation charge per adult, in addition to any cancellation changes imposed by hotels chosen in your itinerary. Airline tickets are non-refundable unless otherwise stated.

REVISIONS/MODIFICATIONS
1. There will be no charge for an initial modification/revision if documents have not been issued.
2. Each additional modification/revision will be assessed a fee of $25.00 per booking.
3. If documents need to be reissued a Document Re-Issue fee of $25.00 will be charged over and above the aforementioned revision fees plus any additional fees imposed by the hotel and/or airlines as selected in your itinerary.

AIRLINE CHANGE FEES
Change Fees of up to $200.00 per ticket may apply and vary by carrier and all tickets are non-refundable unless otherwise stated.

Unused hotel nights are refundable only if they cover a minimum of two consecutive nights and only to the extent that MC&A, Inc. receives a refund from the hotel, after deducting cancellation charges levied by the hotel, if any. Clients are requested to submit written proof from the hotel or other operator that services were not used and are refundable. To be accepted, all refund claims must be presented no later than 30 days after the trip has ended.

CAR RENTAL/TRANSPORTATION
If your Extension Package includes a car rental, a valid driver's license and major credit card are required. All agencies have a minimum age requirement of 25 years of age. Prices do not include taxes, airport fees, gasoline, or insurance.
